For oculoplastics needs, your patient can self schedule online here. Please do not schedule on the behalf of the patient. The patient will need to review our office policies and protocols prior to scheduling.

2) clinical note and reason for consult

3) desired timeframe for evaluation

4) preferred fax number for your office so we can send an appointment confirmation to you as well as our note after the visit

Neuro-Ophthalmology Referrals

Please review the list of procedures performed/conditions treated in our office prior to referring to ensure we can properly address your patient’s problems. Neuro-ophthalmology care in our office is limited primarily to orbital pathology (tumors, thyroid eye disease, proptosis, etc.), facial movement abnormalities (i.e. blepharospasm, hemifacial spasm, facial nerve palsy, aberrant regeneration of the facial nerve, etc.), and ptosis.

We will review records to ensure we can evaluate and treat the condition in question, and we will then schedule your patient. Please send us a fax at 844-456-0925 and include the following information:
